Which drugs appear in the approved 2027 340B Rebate Model Pilot plans, and which related products also appear on that list? This reference brings those names together for 340B program and pharmacy teams reviewing the upcoming pilot.
HRSA currently lists plans from 10 manufacturers, scheduled to take effect January 1, 2027. The reference below includes every brand/formulation name on that approval table and selected related products. It is not a complete inventory of competing medications. HRSA approved plans
Listing products together does not mean they are interchangeable or appropriate for the same patient. This is a product and program-status reference. It does not recommend changing a medication.
How to read the reference
“Listed” means the product appears in the current approved pilot plans for January 1, 2027. “Not listed” means the identified product or labeler does not appear in those plans as reviewed on October 7, 2026. These labels describe the 340B pilot, not insurance coverage, prescribing suitability, or product availability.
The exact 11-digit National Drug Code, or NDC-11, and effective date determine the applicable scope. A brand name, ingredient name, or manufacturer name alone is not sufficient. The federal pilot framework limits participation to qualifying selected-drug NDCs during the applicable period. Federal Register notice, section VIII.C.6
The relationship label identifies a generic's reference brand or a biosimilar's reference product where applicable. “Other medication” does not identify a generic or biosimilar of the pilot drug. FDA's Purple Book separately records biosimilar relationships and interchangeability determinations.

Product identity matters
A generic entry should identify the relevant labeler and reference brand. Apotex reports that its sitagliptin and sitagliptin/metformin products reference Januvia and Janumet, respectively. Par Health separately announced a generic referencing Janumet XR. These entries should not be merged into one unspecified sitagliptin product. Apotex announcement, Par Health announcement
Prasco's fluticasone furoate/vilanterol is labeled as an authorized generic. Its listed package codes, normalized to NDC-11 as 66993013597 and 66993013697, do not appear in the GSK pilot plan reviewed for this reference. That finding applies to those identified products, not to every product containing the same ingredients. Prasco label, GSK approved plan
A biosimilar relationship also belongs to a specific reference product. Hadlima, Yusimry, and adalimumab-aaty reference Humira. Their appearance beside Enbrel or Otezla does not make them biosimilars of those drugs. FDA biosimilar list, Celltrion adalimumab-aaty label
Checking a product's pilot status
Match the exact product, labeler, NDC-11, and effective date to the current HRSA-posted plan. Recheck those sources when a product or plan changes. The same company's products can have different pilot statuses, and two related brands can both be listed.
For example, Farxiga and Jardiance both appear in approved plans. Merck's plan names Januvia, Janumet, and Janumet XR; a Merck manufacturer name alone does not establish inclusion for another product. AstraZeneca plan, Boehringer Ingelheim plan, Merck plan
